In such situations, a negative NT-BNP may avoid some unnecessary echocardiograms. However, keeping in mind the limitations of NT-BNP test, a suggested approach may be that if both the baseline ECG and NTBNP are normal and there is no suspicion of Airways Disease then the patient should be considered for a 6 minute walk and or a trial of diuretics. Echocardiography should then be considered only if either the 6-minute walk or trial of diuretics suggests a diagnosis of heart failure. A practical algorithm for the use of BNP is suggested below. While the taxonomy represents a useful synthesis of the differing approaches, it is relatively unsatisfying from a normative perspective. 154 As I argued earlier, there are dangers in calibrating the intensity of review too precisely and in a manner which does not provide sufficiently distinct judicial methods. It must be doubted whether eight variable standards represents a viable and workable set of standards. The distinctions between the methodology required by these standards are, at best, subtle, and in truth, unrealistic. Supervisory review, particularly of the merits, undeniably involves a strong element of discretion. 155 However, one of the key catalysts for the rethinking of the Wednesbury standard has been complaints about its lack of transparency and confusing nature. The present state of affairs does little to address those concerns and, indeed. A physical, mental, and spiritual device that can enable its users to come into attunement with the cosmic and natural forces in which they are immersed and have their being, and find harmony with their environment and within themselves."3: 34 Within the Medicine Wheel are several different layers of meaning. The Life Force is the power that makes a great oak tree from a tiny acorn or a gigantic grizzly bear from a single egg and sperm. It is the power that makes each of us aware of our own uniqueness that gives us our consciousness and awareness.3 Crucial to the understanding of the Medicine Wheel is the knowledge that Native American cosmology is a science not of materialism but of mind and spirit. All that exists is seen as a manifestation of thought. "Every part of the physical universe and every living thing on the Earth was seen as having its origins not in the material but in the spiritual and mental."3: 36 The physical world is the manifestation of the mind of Shonkwaia'tison--the Creator--imbued with differing amounts of the four primary forces. Even while in manifestation, minerals, plants, animals and humans are in a state of continuous change. "The whole of Nature and of existence was thus regarded as a `coming into being' and a `going out of manifestation, ' and its essence was not material but spiritual and mental."3: 36.
