Propranolol
THE EFFECTS OF PROPRANOLOL ON HEART RATE AND AUTONOMIC AROUSAL IN INSOMNIA Goodrich S, Orr WC Lynn Health Science Institute, Oklahoma City, OK, USA Introduction : Many individuals with chronic insomnia experience high levels of autonomic arousal as compared to individuals with normal sleep patterns. It was hypothesized that a drug that could reduce autonomic arousal might therefore improve the sleep of a group of chronic insomniacs. Methods : Twenty-one individuals with chronic insomnia participated. These individuals completed three polysomnographic PSG ; studies. Heart rate data was also acquired for spectral analysis. The first PSG was for adaptation and confirmation of insomnia. The final two PSGs were preceded by one week of treatment with either propranolol or placebo, in random order. Subjects were treated with either propranolol at 40 mg n 10 ; or 80 mg n 11 ; in order to assess any dose response effect. Spectral analysis of heart rate variability provided an index of autonomic arousal. A series of mixed between subjects by within subjects ANOVAs were conducted on these data. Results : Propranoool significantly reduced heart rate in all stages of sleep and during wake. There was no significant difference between the 40 mg and the 80 mg dose. There was no effect of propranolol on autonomic arousal as measured by the LF HF ratio. Sleep onset latency, sleep efficiency, and other objective sleep measures were not affected by propranolol. Subjective sleep measures of sleep quality and sleep onset latency were not significantly affected by propranolol. Conclusion : Reduction of autonomic arousal, as measured by heart rate and heart rate variability, did not improve measures of sleep in these participants with insomnia. It is likely that hyperarousal is not a general characteristic of insomniacs, but rather a characteristic of a subset of these patients. Future studies that specifically selected insomniacs with hyperarousal could address this issue. Support optional.

AstraZeneca AZ ; is seeking protection for two applications, both providing novel compounds. Among the dozens of pharmaceutical applications made to date in 2006 by AZ, one of the most recent WO2006051290 ; details pharmaceutical compounds for the treatment of cancer. In April 2006, AZ announced an agreement with Abraxis BioScience Inc to co-promote Abraxane for injectable suspension paclitaxel albumin-bound particles ; in the US for a term of 5.5 years. In a randomized phase III study in women with metastatic breast cancer who had failed prior combination chemotherapy, Abraxane demonstrated a significantly higher response rate than standard paclitaxel. Avecia Ltd has applied for protection of an assay. This may indicate a new research direction for the Teeside-based private biotech company, as only a handful of their 84 applications to date appear to be based around diagnostics and assays. The assay may be related to Avecia's advanced tissue engineering project to make ethically-derived adult stem cells for wound healing, announced in April 2006, which will be carried out in collaboration with Durham University, the Blond McIndoe Centre, Smith & Nephew and the Centre for Excellence for Life Sciences. The DOLPHIN database of all pharmaceutical inventions lists 12 records for BIAL all naming Portela as the patent assignee. The School of Pharmacy is seeking protection for a colonic drug delivery formulation. No indication is given as to the location of the school; however, it appears that both KCL and Manchester Universities have recently been investigating this area and it is possible that the present application is the result of one of these universities' research programs, although neither appears to have previously patented on this subject matter. The school of pharmacy, London has published a review of advances in colonic drug delivery in Drugs, 2005, while Manchester's school of pharmacy has had research projects on colon delivery and delaying gastric emptying, under the supervision of Dr J.T. The patient was generally unwell for three weeks prior to the admission with poor appetite but reported no weight loss. Her medication included co-codamol 30 500 ; prn, diclofenac 50 mg tds, risperidone 05 mg nocte, zopiclone 75 mg nocte, domperidone 10 mg tds, proranolol 80 mg bd, citalopram 20 mg od, loprazolam 1 mg nocte, frusemide 20 mg od and loperamide 2 mg prn. The patient smoked ten cigarettes a day, but had no alcohol intake. Physical examination revealed a febrile patient with temperature of 380C. She looked pale, cachectic and dehydrated. An ejection systolic cardiac murmur could be heard at the aortic area radiating to the carotids. There was generalised abdominal tenderness, especially marked at the left iliac fossa, with no guarding or rebound tenderness. Routine blood investigations were as follows: Hb 67 g L, MCV 96 fL, WBC 90 x 109 L, neutrophils 59 x 109 L, platelets 368 x 109 L, albumin 32 g L, urea 88 mmol L, creatinine 148 mmol L. Liver function tests, a coeliac screen, and chest and plain abdominal X-rays were normal. Ultrasound scan of the abdomen only demonstrated gallstones. A barium enema showed an abnormal configuration to the caecum where, lying inferiorly and laterally, there was a lobulated filling defect, the appearances of which were strongly suggestive of a caecal carcinoma. The corresponding CT scan showed fullness at the rectosigmoid junction and raised the possibility of a caecal tumour. Her biochemical parameters deteriorated to the extent that she became too ill for a laparotomy. Total parenteral nutrition therapy was commenced. As no diagnosis had been made, further investigations were performed. Barium meal and follow-through showed a persistent polypoid filling defect in the medial wall of the caecum. A gastroscopy showed a large posterior duodenal ulcer. Colonoscopy was unsuccessful. Therapy was commenced with a protonpump inhibitor and her NSAID treatment was discontinued. She improved sufficiently for an exploratory laparotomy at which a right hemicolectomy and cholecystectomy were carried out. The resected colon showed an ulcer, 20 cm x 10 the ascending colon and a smaller ulcer at the ileo-colic junction. Microscopy Figure 4 ; , showed superficial ulcers, with the mucosa elsewhere showing slight excess of chronic inflammatory cells, occasional distorted crypts, but no cryptitis. No tumour was identified. The aetiology of ulceration was not clear, but ischaemia and IBD were excluded. In retrospect, the features were recognised as consistent with a reaction to NSAIDs. The patient recovered satisfactorily but returned within three weeks with Clostridium difficile diarrhoea. With treatment this settled and she was discharged again and sildenafil and propranolol.
